29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) The #3 palletizer was not guarded at a pinch point created when the table lowers from raised position where employee was making a mechanical adjustment to a guide rail. There was a 65" wide by 69" high opening that provided access to that area. b) The #3 palletizer was not guarded at the pallet feeder conveyor from the magazine or on the top of the table where cartons were aligned to protect employees from the nip points.
