29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about May 15, 2014, at a new home construction site located at 913 Briarwood Ct in Weatherford, OK, no protective devices, effective benching, or sloping were in place to protect employees as they worked in an open excavation, exposing employees to the hazard of being crushed by collapsing soil.
