29 CFR 1910.146(c)(3): When the employer decided that its employees would not enter permit spaces, the employer did not comply with 29 CFR 1910.146(c)(2), and 29 CFR 1910.146(c)(8): 59 Hodges Crossroads, North Adams, MA On May 13, 2014 and at times prior, the employer had not informed employees of the existence and location of permit required confined spaces by posting danger signs and had not developed confined space entry procedures with outside contractors in accordance with 29 CFR 1926.146(c)(8).
