29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: Along the southeast corner of the construction building, on or about April 09, 2012, an employee was using a stepladder that was closed and insecurely positioned along inside a trench approximately 5 1/2 feet deep by 2 feet wide by 52 feet long as an access/egress system into the trench.
