29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used: a) 104 Emerson Court, unit #3, northern most scaffold setup - One employee working approximately 7 feet above the ground, slid down the northern most pole to get down from the working level, on or about 03/05/14; b) 104 Emerson Court, scaffold setup on the south end of building - Employees climbing down from the approximately 12 foot high scaffold platform used an extension ladder that did not extend at least 3 feet above the surface they were climbing down from, above the ground used an extension ladder which, on or about 03/05/14. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
