29 CFR 1910.119(j)(2): The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment: At the establishment, the employer did not develop a written mechanical integrity procedure for inspecting pressure vessels and piping for corrosion under the insulation, thus exposing employee(s) to health hazards due to the possibility of ammonia being released to the working environment. Employee(s) were exposed to release of ammonia vapor / liquid to the atmosphere that historically results in health hazards leading to injury and death to persons in the workplace.
