29 CFR 1926.701(b): All protruding reinforcement steel, onto and into which employees can fall, was not guarded to eliminate the hazard of impalement: a) Two steel tie rods (formwork tie rods) were unprotected, sticking out 15 inches horizontally at eye level in the stairwell just below the top deck. Several rebar were uncapped, sticking up two feet on the top deck near the stairwell. Employees were exposed to impalement hazard/ eye injury. Location: 81 Fleet Place, Brooklyn, N.Y; construction worksite. On or about 2/24/2014 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
