29 CFR 1910.147(f)(3)(i): When maintenance was performed by a crew, craft, department or other group, a procedure was not utilized that afforded the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device: On or about January 29, 2014, Enbridge Pipelines did not utilize a procedure to lock out a section of pipeline that ensured all participants in a valve change were protected against hazardous energy.
