29 CFR 1910.119(e)(1): The employer did not perform an initial process hazard analysis (hazard evaluation) on processes covered by 29 CFR 1910.119: On or about December 13, 2013 in the Biodiesel Plant, the employer did not ensure that an initial process hazard analysis was conducted on PSM-covered processes before start up of the plant. This condition exposed employees to fire and explosion hazards.
