29 CFR 1926.652(g)(2): Excavations of earth material to a level greater than 2 feet (.61 m) below the bottom of a shield was permitted: A. Hank's Plumbing & Heating Co. worksite located at the intersection of Detroit Ave. & Glendale Ave., Toledo, Ohio: On or about December 17, 2013, the employer failed to ensure the bottom of the protective trench shield was within 2ft from the base of the trench. Employees were exposed to potential wall failure when the bottom of the protective shield was approximately 5.00 feet above the bottom of the trench.
