29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: a) Paper Machine #2, at the #2 swing refiner on the operating level - on or about and at times prior to February 05, 2014, the employer did not ensure electric wires and surface mounted equipment such as a metal cased conduit body was installed in a neat and workmanlike manner. The metal cased conduit was deteriorated and lying in water, exposing employees to life safety hazard. b) Paper Machine #2, under the #2 winder in the basement - on or about and at times prior to February 05, 2014, the 120-volt electrical receptacle box was broken, exposing employees to electrical shock hazards. c) Paper Machine #2, at the #4 base refiner on the operating floor - on or about and at times prior to February 05, 2014, part of a metal cased conduit was removed and the electrical wires were left hanging down with caps on the ends, exposing employees to life safety hazard. ABATEMENT CERTIFICATION IS REQUIRED
