29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration; such activities were not performed only by experienced and trained employees selected for such work by the competent person: a) for the ladder jack scaffold sytem used at the gazebo located at 950 Cherry Hills Drive, Sun City Center (FL), on or about 08/07/2013. The owner, who designed and erected the scaffold system: 1) did not assure the width of the platform was at least 12-inches. 2) did not assure the platform was not under the edge of the roof. 3) did not assure the span of the ladder jacks was only 8 feet as per 1926 Subpart L App A - Scaffold Specifications. 4) did not assure the work platform was properly secured.
