Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process: (a) There was no MOC (management of change) conducted following a 7/8/11 process flash crude release in the DU1 unit at the P3683 suction pump one inch three hundred pound seal flush piping which precipitated subsequent line replacement. The threaded components of the auxiliary process liquid piping were replaced with a different alloy tubing material with expansion loops while a flange and orifice assembly, dial gauge, and strainer were removed from the circuit. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
