29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) John Sheldon dba J & S Siding, Bloomdale, Ohio: On or about July 15, 2011, at address 822 W. Bigelow, Findlay, OH, the employer did not ensure that the side rails of an extension ladder were extended at least three feet above the landing. Employees were exposed to falls while using the ladder to access the working level to perform roofing activities.
