Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure: a) Plant - On or about 07/24/13 employees applied lockout/tagout for maintenance and repairs of equipment including (but not limited to) the green TSInc foam grinder, the blue Fabtec foam grinder, and Molding Machines 1, 2, 3, and 4. Affected employees were not trained on the control of hazardous energy and lockout/tagout. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
