29 CFR 1910.23 (a)(9): The employer did not guard the floor holes into which persons cannot accidentially walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securlely held in place to prevent tools or materials from falling through: (a) Cyclone Drilling, Inc., Drilling Rig 17, API 05-045-19478, Rifle, CO 81650: On and before July 7, 2011 the creating, exposing and correcting employer did not guard six circular openings of approximately 9'' diameter on the mud tank floor. These hole openings have 14'' rods sticking out of them as well. This condition exposed employees to slip, trip, and fall hazards. Abatement Note: Abatement certification and documentation are required for this item (see enclose Corrective Action Worksheet).
