Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.178(n)(8): Under all travel conditions the truck shall be operated at a speed that will permit it to be brought to a stop in a safe manner: (a) On or about May 15, 2013, an employee operating a Toyota Stand-up Forklift, lost control of the forklift and ran into the storage racks. (b) On or about July 6, 2013, an employee operating a Toyota Stand-up Forklift, lost control of the forklift and ran into the storage racks. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
