29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe and defective rigging equipment was not removed from service: On or about June 22, 2013 a 5/8 inch diameter wire rope sling, serial number 145270-23, 3.9 ton capacity had a hook safety latch that did not close. The sling was used to lift rafters, exposing employees to the hazards of being struck by falling materials.
