29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired. a) Roof bracket scaffold, roof, Old Zion Church, 628 North Broad Street - Employees were performing roofing work from a roof bracket scaffold that was erected with support bracket spacing which exceeded the maximum allowed span. This weakened scaffold was not immediately repaired, replaced, or braced or removed from service until repaired. Observed on or about 6/21/2013. b) Roof bracket scaffold, roof, Old Zion Church, 628 North Broad Street - Employees were performing roofing work from a roof bracket scaffold that was erected and attached to the roof with fewer and smaller nails than required. This weakened scaffold was not immediately repaired, replaced, or braced or removed from service until repaired. Observed on or about 6/21/2013.
