29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Bay #1 Punch: Employees used a Peddinghaus 225B/20 full revolution punch #FH90 to punch holes in metal sheet. The punch was not guarded to protect the operator or other employees in the area from the point of operation or flying chips that are being punched out of the metal, as determined on 5/30/13. b. Bay #3 Ironworker: Employees used a Peddinghaus 110/140 Ironworker #442364660206036 to punch metal sheet. The punch guard was damaged with a 2-1/2 inch vertical x 1-1/8 inch horizontal opening exposing the operator or employees in the area to the point of operation or flying chips punched out of the metal, as determined on 5/30/13. c. Oster threading machine: Employees use an Oster 784 XZ-27 Pipe & Bolt machine to thread metal stock. The rotating chuck at the rear of the machine had protruding jaws and was unguarded, as determined on 5/30/13. Abatement Certification Required For This Item.
