29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device were not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, were not designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: A. Plasto-Tech Corporation worksite located at 708 Lowell St., Elyria, Ohio: On or about May 24, 2013, the employer did not ensure the Brown Thermo-Forming press was adequately guarded including but not limited to the lack of perimeter guarding around the turntable, guarding at the loading station, and guarding at the mold station. Employees were exposed to caught between and struck by hazards while operating the press. B. Plasto-Tech Corporation worksite located at 708 Lowell St., Elyria, Ohio: On or about June 25, 2013, the employer did not ensure the Brown Thermo-Forming press was adequately guarded to protect the operator at the loading station. The employee was exposed to caught between and struck by hazards while operating the press.
