29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by flying chips and debree: (a) Maintenance Department : The employer failed to ensure that a guard was provided to prevent flying chips and oil mist from reaching an employee when operating the South Bend Precision lathe. (b) Mainteance Department : The employer failed to ensure that a chip guard was provided to prevent flying chips from reaching an employee when operating the Supermax Series-II milling machine.
