29 CFR 1926.1053(b)(6): Ladders were not used on stable and level surfaces nor were they secured to prevent accidental displacement: a) On or about 05/07/13 CSHO observed employees using a Werner, 40 foot aluminum extension ladder to access the roof when the ladder was not secured nor was it used on a stable and level surface.
