29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, amputation, and pressing hazards: a. On or about April 21, 2013, in the walkway on lane six (6), there was an 18 inch by 11 inch opening, exposing employees to the hazard of getting crushed by the mechanical table or the mechanical sweep. b. On or about April 21, 2013, in the walkway on lane seven (7), there was an 18 inch by 11 inch opening, exposing employees to the hazard of getting crushed by the mechanical table or the mechanical sweep. c. On or about April 21, 2013, in the walkway on lane twelve (12), there was an 18 inch by 11 inch opening, exposing employees to the hazard of getting crushed by the mechanical table or the mechanical sweep. d. On or about April 21, 2013, in the walkway on lane thirteen (13), there was an 18 inch by 11 inch opening, exposing employees to the hazard of getting crushed by the mechanical table or the mechanical sweep. e.
