29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: a) Utility Work Site: On or about April 26, 2013 and at times prior, employees were exposed to a trench cave in while replacing a concrete fiber optic vault in an excavation approximately six foot, seven inches deep that had not been inspected by a competent person for hazards.
