29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a. On or about 4/23/13, West Tower of Standard Elevator 2, a fall protection harness and lanyard attached to an aerial lift anchorage point was not worn by an employee in the aerial lift who tightened bolts of the west tower thereby exposing the employee to falls from heights 6 feet or greater.
