29 CFR 1910.269(d)(6)(iii): All energy isolating devices needed to control the energy to the machine or equipment were not physically located and/or operated in such a manner as to isolate the machine or equipment from energy sources: At the AMEREN-UE Calloway Nuclear Plant, switchyard, specifically at Bus B, 52-4, an arc flash occurred on or about 04/02/13 severely burning employees. All of the electrical energy isolating devices required to isolate Bus B, 52-4, were not physically located and/or operated prior to the employees attempting to place personnel protective grounds on connectors located inside of this piece of equipment.
