29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. Dry lines - On or about April 4, 2013, an employee performing clean out servicing tasks on the dry line mixer #6 did not affix a lockout or tagout device. Employees were thereby exposed to the hazards associated with the unexpected energization of equipment. b. Plastic Lines - On or about May 6, 2013, employees who performed clean out servicing tasks on the plastic line #4 conveyor belt chute did not affix a lockout or tagout device. Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.
