29 CFR 1910.119(j)(4)(ii): Inspection and testing procedures did not follow recognized and generally accepted good engineering practices: The employer does not ensure that inspections and tests that are performed to maintain the mechanical integrity of process equipment follow recognized and generally accepted good engineering practices (RAGAGEP). This violation occurred on March 28, 2013, and times prior thereto, in the Blowing Agent Storage Are; where the employer, by not ensuring that inspections and tests were performed on pressure vessels and piping in accordance with RAGAGEP such as API 510 and API 570, exposed employees to fire and explosion hazards. Identified vessels and piping included but were not limited to the Propane and Butane (Blowing Agent) Process Vessels and Piping.
