29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. Where machines were maintained, adjusted, repaired, and serviced, such as, but not limited to, changing saw blades, a lockout program was not developed and implemented, including identifying machines requiring to be locked out during servicing, where and how to lockout, how to test to ensure machine is locked out, providing training to both employees that perform lockout but also those who work on equipment that could be locked out. (Note: training documentation is required, as is an annual inspection or audit of the lockout program to ensure procedures are followed, employees are locking out as required, and lockout equipment is available, such as individually keyed locks and multiple hasps.) Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
