29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: a) 1718 North 42nd Street, Philadelphia, PA- On December 18, 2012 employees erected a tubular steel frame scaffold on the front sidewalk. There was no competent person qualified in scaffold erection supervising this work. The upper steel frame was 20 inches from the closest phase of the 13.2KV distribution service and the planked working level was 28 inches away and at the same level. Work began from the top level over 32 feet above the sidewalk below and no standard guardrails were installed or other fall protection provided utilized. Conditions noted on December 19, 2012. NOTE: Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
