Text For Citation: 01 Item/Group: 003 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Upper level of mix house, dept. 077, between #1 and #2 cross-overs on track 31, northeast side of building, where employees from the traffic dept. 347 pushed railroad cars with locomotive in an unsafe manner, in that conductor did not maintain clearance and controlled movement before pushing cars on track, Plant Safety Procedures #24, item #6 and #8, traffic dept. 347, Safety Rules Section "A", page #2, item #24 and JAS #22, clearance and controlled movement was not maintained while employees in another department were in the area prior to car movement, thereby exposing them to crushing injuries between moving cars and parked locomotive (Mix Mobile) on the same track, on or about January 1, 1987.
