29 CFR 1926.1430(d): The employer did not train each competent person and each qualified person regarding the requirements of this subpart applicable to their respective roles. On or about July 5, 2012, the employer did not ensure that competent persons and qualified persons involved in the placement of bridge girders over Lake Butte des Morts, Bridge B-70-266, were trained on the requirements associated with multiple crane lifts under C.F.R. 1926.1432. Employees were thereby exposed to the hazards associated with crane and load failures.
