29 CFR 1915.152(e)(1): The employer did not provide training to each employee who is required, by this section, to use PPE: a/ The Elevator Man, Inc./New York Container Terminal, Staten Island NY Dry Dock Rio De Janiero Express Ship Site: Training was not provided to ensure employees used personal protective equipment when required. On or about 6/16/2012 fall protection was not used where employee(s) were required to perform maintenance operations aboard the "Rio De Janiero Express" cargo container ship elevator, located at the New York Container Terminal, Staten Island NY dry dock. Employees were on top of the elevator at a height in excess of 30 ft.
