29 CFR 1926.1404(a)(1): Assembly/disassembly was not directed by a person who meets the criteria for both a competent person and a qualified person, or by a competent person who is assisted by one or more qualified persons ("A/D director"). A crew of six (6) workers were exposed to crushing and struck-by hazards during the assembling of a 118 Link Belt Crawler Crane that was under repair. This assembly was conducted without the supervision of a competent person. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
