29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: This violation occurred on May 03, 2012, at a jobsite located at 4047 Childress Avenue in Mesquite, Texas 75150, where the side-rails of the ladder that at least eight (8) employees used to access/egress the two story apartment building did not extend at least 3-feet above the upper landing surface.
