29 CFR 1910.119(d)(3)(i)(E): Process safety information pertaining to the equipment in the process did not include the ventilation system design requirements: a) On or about February 24, 2010 - the PSI did not contain information that was needed to ensure that the machine room ventilation air flow paths would prevent the accumulation of ammonia vapor in air at concentration greater than 16 percent. Additionally, the employer did not follow good engineering practice requirements for using non-manual, fail open air intake louvers and exhaust fans that were located in a manner that would promote air flow throughout the machine room and avoid short circuiting of ventilation air currents as specified in IIAR Bulletin 111-1991, Sections 5.1 and 5.2.
