29 CFR 1910.119(f)(4): The employer did not implement safe work practices for employees and contractor employees to provide for the control of hazards during operations such as lockout/tagout, confined space entry, and opening process equipment or piping: 21178 SW 89th Rd., Beatrice, NE 68310 (CO2 Stripper): The employer did not implement safe work practices to provide for the control of hazards during operations such as lockout/tagout, confined space entry, and opening process equipment or piping, including but not limited to, the CO2 Stripper. The procedure for clearing the CO2 Stripper had been developed but not fully implemented prior to allowing contract employees to begin work on it. The vessel had not been inerted with nitrogen. A contract employee incurred serious burn injuries when an explosion occurred while he was working on the 105C Reboiler. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed.
