Text For Citation: 01 Item/Group: 003 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crushing injuries from overhead cranes that had malfunctioning bridge brakes and hoist controllers. a.Republic Engineered Products, Inc. located in Lorain, Ohio: On or about May 13, 2010, the employer did not ensure the #4 wire crane located in the Conditioning Building was equipped with functioning bridge brakes. The brakes were not allowing the crane to stop. b.Republic Engineered Products, Inc. located in Lorain, Ohio: On or about May 13, 2010, the employer did not ensure the #4 wire crane located in the Conditioning Building was equipped with a properly functioning hoist controller. The hoist controller allowed loads to be dropped by the hoist. c.Republic Engineered Products, Inc. located in Lorain, Ohio: On or about May 13, 2010, the employer did not ensure the #3 wire crane located in the Conditioning Building was equipped with functioning bridge brakes. The bridge brakes were not allowing the crane to stop. Among other methods, one feasible and acceptable method to correct this hazard is to ensure cranes are removed from service when deficiencies are identified in their safe functioning according to ASME B30.2-2005 Overhead and Gantry Cranes (Top Running Bridge, Single or Multiple Girder, Top Running Trolley Hoist). In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation ( using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
