29 CFR 1910.119(d)(3)(ii): The employer did not document that the equipment in t he process complied with recognized and generally accepted good engineering prac tices: SAVE: 29 C.F.R. 1910.119(d)(3)(ii): The employer did not document that eq uipment complies with recognized and generally accepted good engineering practic es such as, but not limited to, API Recommended Practice 521. This hazard was ob served on or about May 10, 2010. Two intervening valves are not carsealed or loc ked open as required by recognized and generally accepted good engineering pract ices, such as, but not limited to API Recommended Practice 521. Nor were they lo cked open as required by Calumet Procedure 00-H-0138. These valves are located i n the Platformer Unit between the Separators and RV 18-06 and RV 19-04. If inter vening valves are closed, relief valves are rendered ineffective. This condition exposes employees to fire and explosion hazards. Pursuant to 29 C. F. R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation that it is in compliance with the standard, including describing the steps it is taki ng to ensure all intervening valves are locked open, and verified so by regular inspection. One method of abatement among others is to initiate a program to ins pect all relief valves on a regular basis.
