Text For Citation: 01 Item/Group: 003 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads: (a) Facility, New Castle Road, Butler, PA: On or about February 3, 2010, there were no frequent (monthly) inspections being performed, with written reports, on the three five ton hoists. (b) Facility, New Castle Road, Butler, PA: On or about February 3, 2010, the last periodic (yearly) inspection performed on the three five ton hoists was performed March 12, 2008. Among other methods, one feasible and acceptable means of abatement for this violation is to perform monthly and yearly inspections in accordance with the manufacturer's recommendations and the American Society of Mechanical Engineers, ASME B30.16, Section 16-2.1.2; Section 16-2.1.3; and Section 16-2.3.1.
