29 CFR 1910.146(d)(5)(ii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not evaluate permit space conditions when entry operations were conducted by testing or monitoring the permit space as necessary to determine if acceptable entry conditions were being maintained during the course of entry operations: Employees entered tank 242 under a confined space permit to perform mantenance and cleaning. Although air monitoring was done during this work at the tank entry opening, no testing was done in the area of the tank where employees were removing sludge and cleaning the tank floor.ng
