Text For Citation: 01 Item/Group: 003 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: (a)Integrity Trucking LLC at Merit Well Girltey 31-23 WCR 38 - 25.5, Gilcrest, CO 80623: On or before October 24, 2008, the employer did not ensure that, when applying static pressure with the base-mounted winch on the 2000 Freightliner Classic truck, serial number 1FUPCXZB6YPF04439: prior to the operator leaving the controls, the operator and an appointed individual did not establish the requirements for braking, dogging the hoist, setting up barricades, or whatever other precautions may be necessary. With only one employee responsible for the operation of the hoist and the alignment of the holes used to support the roller bar, there was no systematic way to ensure the alignment of the holes, while applying static pressure, prior to the operator leaving the controls. Condition exposed employees to struck by hazards. Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter"). Among other methods, one feasible and acceptable abatement method can be found in the American National Standards Institute ASME B30.7 - 2001 Section 7-3.2.3, which provides that when applying static pressure, prior to the operator leaving the controls, the operator and an appointed individual shall establish the requirements for braking and dogging the hoist, furnishing notices, setting up barricades, or whatever other precautions may be necessary.
