Text For Citation: 01 Item/Group: 003 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Acts of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards: a.Drilling Site. Employees were exposed to the hazard of collapse of the derrick/mast in that the rig contractor did not successfully inspect surface ground conditions that might indicated reduced anchor capacity, in that the deadman anchor/stakes were located approximately 43 inches from a ditch/spring of water, with the water seeping from a bank that was located next to the ditch. These conditions could contributed to failure of a deadman anchor/stakes. Employees were performing work off of the rig floor and pipe racking board on the Rambler Rig #21, serial # 689, as determined on or about 05/17/2008. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO BE MORE THOROUGH IN INSPECTING THE SURFACE GROUND CONDITIONS THAT MIGHT INDICATE REDUCED ANCHOR CAPACITY, AS SPECIFIED IN THE RECOMMENDED PRACTICE FOR USE AND PROCEDURES FOR INSPECTION, MAINTENANCE, AND REPAIR OF DRILLING AND WELL SERVICING STRUCTURES (API RECOMMENDED PRACTICE 4G), SECTION 14.5 (D).
