Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Halliburton Energy Services, Dora Roberts Devonian Unit 3401, Midland County, TX. On 11/21/07, the employee operating and monitoring a HT2000 3512B Pump on a manifold truck (Unit #10864618) to set a pump seal was unable to recognize potential hazards of inadvertently pressurizing the system without verifying it was safe to do so. Among others, feasible means of abatement could include: Ensure all employees are properly trained before they are allowed to work unsupervised during critical operations.d
