Text For Citation: 01 Item/Group: 003 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to exploding parts during the pressurization of a double acting hydraulic cylinder: (a)Primary Mill. While assembling a table roll for the 110 inch mill, one of two required hydraulic hoses were attached to a double acting Simplex hydraulic cylinder, Model XR1006AD, S/N 217, powered by a Power Team porta power unit, Model PE 554T, S/N 319259. At an unknown pressure, an Enerpac hose coupler, Type CR40003994X, attached to the unused return port failed, releasing internal parts, on or about May 28, 2007. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure all double-acting cylinders are fitted with a relief valve, as specified by Chapter 1-1 Section 1-1.1.3 (b)(2) of the ASME B30.1 - 2004, Safety Standard for Jacks.
