Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to high pressure hoses and flow lines not secured to prevent whipping action in the event of failure: a) Jones Unit 3, Woodcock Twp, Crawford Cnty, PA - On or about June 12, 2008 two (2) three inch (3") high pressure hoses were not hobbled with safety chains to prevent whipping action in the event of hose failure. b) Jones Unit 3, Woodcock Twp, Crawford Cnty, PA - On or about June 12, 2008 the main flow line was not secured with tie downs (chain and binder) on each side to prevent movement.
