Text For Citation: 01 Item/Group: 003 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: (a) Location - Dye house: On or about 11/3/05, employees were exposed to crushing hazards while utilizing a 4000-pound capacity Wright-Speedway hoist which was mounted to a 2000-pound capacity steel monorail. Among other methods, one feasible and acceptable means to abate the hazard is to install a properly-rated hoisting mechanism in accordance with ASME/ANSI B30.16.2003.
