Text For Citation: 01 Item/Group: 003 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-between and struck-by hazards: a)A-Du Well Auto Salvage & 5135 Emerson St. in Denver, CO: The employer did not ensure rough terrain forklift trucks were inspected prior to use and removed from service when unsafe conditions existed such as but not limited to a missing gas cap, a missing engine cover, a cracked windshield, hydraulic fluid leak, broken operator's panel, tires in poor condition, and broken lights. This condition exposed employees to caught-between and struck-by hazards. Among other methods, one feasible and acceptable abatement method to correct this hazard is to perform the following: ASME B56.6-1992, American Society of Mechanical Engineers, Section 6.5.1.1 "If the rough terrain forklift truck is found to be in need of repair or in any way unsafe, or if it contributes to an unsafe condition, the matter shall be reported immediately to the user's designated authority, and the truck shall not be operated until it has been restored to safe operating condition. NOTE: The ASME B56.6-1992 Section 6.2.7 standard can be obtained by contacting the American Society of Mechanical Engineers, 345 East 47th Street, New Tork, NY 10017. b)A-Du Well Auto Salvage & 5135 Emerson St. in DEnver, CO: The employer did not ensure the rough terrain forklift truck was inspected prior to use. This condition exposed employees to struck-by hazards. Among other methods, one feasible and acceptable abatement method to correct this hazard is to perform the following: ASME B56.6-1992, American Society of Mechanical Engineers, Section 6.5.1 "At the beginning of each shift and before operating the rough terrain forklift truck, check its condition, giving special attention to": 1)Tires and their inflation pressure; 2)Warning devices; 3)Lights: 4)Lift and tilt systems, load-engaging means, chains, cables, and limit switches; 5)Brakes;6)Steering mechanism; and 7)Fuel system(s).
