Text For Citation: 01 Item/Group: 003 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to entanglement hazards: (a) SoundTrack Audio & Video, at 1370 S. Colorado Blvd., Denver, CO 80222: On or before March 16, 2004 the employer did not ensure that there was an audible and visible warning alarm indicating that movement of the "upbelt" conveyor was initiating. Employees riding the conveyor and those working in close proximity to the conveyor were exposed to fall hazards and entanglement hazards as they cleared materials jamming the conveyor. (b) On or before March 16, 2004 the employer did not ensure that there was an audible and visible warning alarm indicating that movement of the "downbelt" conveyor was initiating. Employees riding the conveyor and those working in close proximity to the conveyor were exposed to fall hazards and entanglement hazards as they cleared materials jamming the conveyor. One feasible method of abatement, among other methods, is to install horns and strobe lights, which are linked to the conveyor activation switch, and which allow for a delayed response. These warning devices shall be installed in accordance with industry consensus standard ASME B20.1-2000 5.11.2(b)(1)(d).
